  deniedDomains in v2.1.113)
- effortLevel: add xhigh (Opus 4.7, v2.1.111) and fallback note
- add CLAUDE_CODE_ENABLE_AWAY_SUMMARY env var
- bidirectional cross-links between settings report and
  claude-cli-startup-flags.md for CLAUDE_CODE_SIMPLE /
  CLAUDE_CODE_EFFORT_LEVEL
- quick-reference example uses the new keys
